What to Bring
The patient must present a legal form of identification upon check-in. Valid identification includes a driver’s license, passport, military ID, employment badge or school ID card.
The patient must present a document that has their current proof of residence This may include Property Tax Receipt, Posted Mail with name of applicant, Utility Bill, Lease Agreement, Insurance Card, Voter Registration Card, College Enrollment Papers.

Please print out the documents for your review and/or completion and bring them with you at the time of your first appointment.
Payment Options
NCIAP Medical Care prides itself in being a faster, more convenient and affordable option for your urgent medical needs. We accept:

- Checks (If your check is returned by your bank for insufficient funds or for any other reason, a $30 service charge will be charged to your account.
We will then ask you to pay for all future visits by cash or credit card.)
The typical office visit cost is $55.00, in some cases additional lab work is needed to determine the exact cause for the illness, in which case there would be an additional $10 fee for this service. If any other diagnostic procedures are needed for diagnosis, the patient is encouraged to inquire about fees at the time of service.
